So today when I was holding my snake I notice that there is like a piece of skin that looks like it's ripping off at the top of the head and on his body it looks like a scale is coming off or something, and it's hard to take a picture cause he's always moving lol

Is there something in his viv that could have injured him? Can you take a clear picture of it? I guess, if it is a whole, real scale, I would snip it off so it doesn't tear further, and put antibiotic ointment on it if you have any.

No it's like smaller then his scale

It's like see through and like maybe around a month

maybe it just from his last shed and you didn't notice it till now, it's happened to my corn a couple of sheds back. It will come off on his next shed.
