I have done this survey for these people for two years now. :) They do dogs, cats and ferrets. I'll post the link for cat/ferret ones too, just incase! :)

You only have until the end of October to finish these before the study is done! Its neat cause you get an email once they're finished showing stats and stuff.
 
Filled it out for my two pups and my brother's (well, let's be honest, I take care of her..) ferret. Neat little survey.

I really liked the option to enter information about previous diets, and health improvements related to the diet change. The ferret came to us on crappy Walmart food, and both the dogs were on Science Diet (which I now know is crap) most their lives, so the opportunity to say "this is what they were on, and these are some of the significant health improvements since the change" was very nice for me.
