FedEx certified to ship process

Last week (May 22, 2013) I sent my test package out with the application. How long does it usually take for them to email you your results? Thanks.

It's been several years since I did it, maybe they've gotten better, but I had the absolute worst rep ever. So stay on top of them if you don't hear something in a week or so.

It took me several months to get certified - which is not normal - but can happen if you get someone incompetent.

Ok so I got an email yesterday stating that my package passed(yay!). I then got another email today from a FedEx sales rep with a waiver that I have to sign. But I don't know what to put for two of them. It looks like this

What do I put for "by" and "title" ? Also, can I type all the spaces in, or do I have to print it out and write it all in? Thanks for any help.
