I will be 100% honest here... I have no idea what the heck that thing is, or even how to beginning researching that! Whatever it is, looks like it didn't stop fast enough and smacked its face into a wall lol!

I did a search under pin-head frog, and amazingly it didn't work. I have no idea how to find this one either!
 
They are tiny, and under moist rotting logs, often several. The first time I found one, I really thought I had discovered something. (I could give you more obvious hints, but you're already stating a few.)
 
Noronha worm lizard? I dont know the scientific name, All I can say is that i knew it was a worm lizard. Is it keel headed or shovel headed?
 
Yeah, I just did a quick search, your right, you take it though, I only have corn snake pictures :p
